Have any players in your family been granted a WYSA/CFSA soccer scholarship in the past 4 years? If so, tell us the player(s) name and scholarsip amount(s).

Select a scholarship option based on the portion of the registration fee you are able to pay. The partial registration fee you pay covers insurance, use fees, state fees, and the cost of the jersey. *****PLEASE NOTE: All recipients are expected to volunteer during the season they've been granted the scholarship. A member of the soccer board will contact the parent/guardian to schedule hours.

40% Scholarship (you pay 60% of the registration fee). NOTE: Recipients are expectd to volunteer 6-12 hours per season.

75% Scholarship (you pay 25% of the registration fee). NOTE: Recipients are expected to volunteer 6-12 hours per season.

90% Scholarship (you pay 10% of the registration fee). NOTE: Recipients are expected to volunteer 12-24 hours per season.

If approved, you are or your player is expected to provide volunteer time for CFSA activities. Please acknowledge this expectation by checking the box.
